We are currently seeking a part-time, 32 hour Registered Nurse, a full-time, 40 hour nurse and a per diem nurse to work in our Adult Medicine Department in Woonsocket and in our West Warwick locations.
General Responsibilities:
Provides clinical and technical support for Health Care Providers (i.e. immunizations, injections of antibiotics). Provides high quality clinical and customer service response by promptly prioritizes triage call and using previously established practice guidelines in order to offer advice and health information. Coordinates sick visits/add-ons as needed. Acts as a resource person for medical assistants and other team members. Coordinate prescription refill. Reviews and categorizes diagnostic tests and provides follow up as needed.
Required Qualifications:
Current State of Rhode Island Registered Nurse license
Three or more years nursing experience
Ambulatory Health Care experience preferred
HIV certification desirable
Ability to communicate effectively and work collaboratively with patients, providers, other members of the health care team, other disciplines and community agencies.

Supervision and Management:
· Recruit, hire, train, supervise, and evaluate Outreach and Enrollment Workers; provide indirect supervision and training to other staff conducting outreach and enrollment activities.
· Provide training on the Affordable Care Act (ACA) and the enrollment process to Thundermist staff.
· Assist in the development of the Thundermist’s Outreach and Enrollment Plan and associated “enrollment” and “assistance” goals.

Enrollment and Outreach:
1. Provide information to consumers regarding the ACA as it relates to the health insurance coverage options for Rhode Islanders. This information includes:
· Program eligibility: rules to purchase subsidized insurance through the Exchange and eligibility for Medicaid, CHIP, Medicare or other programs;
· Methods of purchase: provide information concerning enrollment in Qualified Health Plans, premium tax credits, and cost-sharing reductions
· Explaining definitions of health insurance terms, for example, aiding the consumer to understand the difference between a premium, deductible and co-insurance;
· Answering enrollment questions and addressing questions regarding access to any of the enrollment methods and the submission of enrollment documentation to the Exchange;
· Explaining eligibility criteria for purchasing insurance through the Exchange, enrolling in Medicaid and other State programs designed to provide medical coverage;
· Furnishing unbiased explanations of coverage provided on the Exchange portal without offering any opinion or editorial on the QHPs in the Exchange.
· Providing the consumer with documentation regarding the available plans, enrollment letters stating the date coverage will start, etc.;
· Providing information to the consumer to find avenues to resolve disputes with carriers;
· Facilitate enrollment in QHPs
· Provide culturally and linguistically appropriate health insurance enrollment assistance for Hispanics, Asians, Native Americans, those with disabilities, homeless individuals, LGBT individuals, and other vulnerable populations.
Key Qualifications:
· Bachelors Degree required in health care, social work, education, sociology or psychology or other relevant field of study (5- plus years outreach experience and exceptional written communication skills may be considered as meeting the educational requirement).
· Masters Degree in relevant field of study preferred
· One year of supervisory experience required, two-plus preferred
· Two years experience working with vulnerable populations in a community setting preferred; willingness and comfort working with low income people required
· Knowledge of state and federal health care programs, including Medicaid and other public benefits programs preferred
· Ability to maintain strict confidentiality of consumers’ private financial and other information
· Excellent oral and written communication skills; bilingual (Spanish or Laotian ) preferred

Coordinate sick visits or additional services as needed.
Required Qualifications:
1. Current State of Rhode Island RN license
2. Experience in community health setting , public health, chronic disease management, community nursing, case management preferred
3. Experience in working with primary care providers in practice setting to coordinate care and disease management.
