You can use this form to request a letter of documentation from the Rhode Island Department of Health (RIDOH), confirming your status as it pertains to COVID-19. There are several different types of letters RIDOH will issue including:
- Quarantine letter
- Release from quarantine or isolation letter
- Tested positive letter
- Tested negative letter
- Parent/child quarantine letter
- Exemption letter (when employer is requesting a negative test before return to work)
RIDOH will send the requested letter to your email address within 72 hours.
